Transcranial Electrical Motor Evoked Potentials (tceMEP) in Anterior Cervical Discectomy and Fusion Surgery: A Comparison Study between Automated and Non-Automated Technologies

چکیده

INTRODUCTION Spinal cord injury resulting in postoperative motor deficits is a serious complication of major spine surgery. Intraoperative electrophysiologic monitoring is performed to assess the functional integrity of the spinal cord in order to recognize and avoid injury. Motor pathways have been successfully monitored using transcranial electrical motor evoked potentials (tceMEP or MEP). A novel system offers automated intraoperative monitoring capabilities that utilize the “threshold-level” alert criteria method which provides more detailed myotome-specific responses, contrasted with the conventional “presence-or-absence” technique which uses a single supramaximal stimulus to obtain responses from one or all myotomes. The automated system then displays this feedback to the end user in a simplified form. The purpose of this study was to compare the automated system to that of conventional, nonautomated technology.

Neurophysiological monitoring of spinal cord function during instrumented anterior cervical fusion.

BACKGROUND CONTEXT Somatosensory evoked potentials (SSEPs) monitor global spinal cord function, and the interpretation of motor loss is based on inferred rather than direct measurements. Therefore, SSEPs may not be useful for identifying motor function deficits caused by anterior spinal column injury or nerve root injury during decompression or placement of instrumentation. For these reasons, a...

Loss of transcranial electric motor evoked potentials during pediatric spine surgery with dexmedetomidine.

Case report: Intraoperative neurophysiologic monitoring using transcranial electric motor evoked potentials (TceMEP) has been increasingly utilized to reduce the risk of spinal cord injury during corrective spine surgery. Since inhalational anesthetic agents considerably depress TceMEP amplitude in a dose dependent manner (1,2), total intravenous anesthesia (TIVA) techniques with propofol as a ...

Comparison of outcomes and safety of using hydroxyapatite granules as a substitute for autograft in cervical cages for anterior cervical discectomy and interbody fusion

Background:   After cervical discectomy, autogenetic bone is packed into the cage to increase the rate of union between adjacent vertebral bodies, but donor site–related complications can still occur. In this study we evaluate the use   of hydroxyapatite granules as a substitute for autograft for interbody fusion.     Methods:   From November 2008 to November 2011, 236 patients participated in ...

Two cases of quadriparesis following anterior cervical discectomy, with normal perioperative somatosensory evoked potentials.

Two cases illustrate an uncommon failure of perioperative somatosensory evoked potential (SEP) monitoring to detect iatrogenic lesions causing temporary quadriparesis during straightforward cervical surgery. In both cases, anterior cervical discectomy at one or two levels was undertaken with bone graft and titanium implants, and cortical SEP were monitored to alternate stimulation of the left a...

Bilateral C5 Palsy Following a Circumferential Surgery for Cervical Spondylotic Myelopathy: Case Report and Review (In press)

Background and Importance: The development of C5 root palsy is a well-known potential complication of cervical spine surgeries for the correction of cervical spondylotic myelopathy. It typically occurs unilaterally but on extremely rare occasions it might occur as a bilateral anomaly. The value of intraoperative neurophysiologic monitoring in detection of iatrogenic acute versus delayed onset C...
